Health Connections Clinic

Health Connections Clinic at the Comox Valley Nursing Centre is team-based primary health care providing longitudinal care for Comox Valley residents who don't have a regular family doctor or nurse practitioner.
They specialize in comprehensive care for people living with complex medical and/or socio-economic needs. These can include mental health, substance use, homelessness, poverty, minorities, Opioid Agonist therapy, safe supply, and/or complex chronic disease management.

They are family doctors, nurse practitioners, social workers, registered nurses and administrative staff who work with you as a part of your team to best support your health needs and priorities.

To access Health Connections Clinic:

* Patients can call or drop in to fill out our waitlist consideration form (note: all people without primary care should also place themselves on the Comox Valley Health Connect Registry).

* Providers can refer unattached patients by using the Comox Valley Island Health Single Referral Form]

The Health Connections Clinic is unable to accept referrals for patients who are already attached to a primary care provider in the community.
