Low barrier, multi-interdisciplinary team-based primary care clinic that provides longitudinal care for Comox Valley residents who don't have a family doctor or nurse practitioner.
The Health Connections Clinic is located in the Comox Valley Nursing Centre, specializing in comprehensive care for people living with complex medical and/or socio-economic needs. These can include mental health, substance use, homelessness, poverty, and/or complex chronic disease management.
Healthcare is provided by family doctors, nurse practitioners, social workers, registered nurses and administrative staff who work with the patient as a part of a team to best support health needs and priorities.

How To Access Health Connections Clinic:
- IF the patient does not have a family physician or nurse practitioner
- Patient can call to book an initial intake appointment with a primary care nurse (Note - anyone without primary care should also place themselves on the Comox Valley Health Connect Registry
- Agencies and other providers can refer unattached patients by using the Comox Valley Island Health Single Referral Form
- Patients may call or drop-in to book follow-up appointments as needed, or with inquiries

Hours of Operation are Monday to Friday, 8:30 AM - 6:00 PM. Clinic is closed daily for lunch from 12:00 PM - 1:00 PM, as well as on all weekends and statutory holidays.
